Before You Enroll

Enrollment is based on program availability, your child’s age, schedule, and service needs. Families may be asked to provide required health, emergency, identification, and enrollment information before care begins.
Required Forms & Documents

Families must complete all required enrollment forms and provide any documentation needed for licensing, health, emergency contact, authorization, transportation, or other program requirements. Some forms may vary depending on the child’s age or services requested.
